If you are concerned that your memory, or the memory of a loved one or friend is not what it once was, then you may well be looking at this site to learn more and seek out how to get some help.

Whether you have a formal diagnosis, or not, its good to find out more about the help that is available as soon as possible. Following a diagnosis it is really important to everyone to have follow-up advice and guidance and support. The sooner this starts the sooner you will find your ways to manage, to cope, to get through and so to live better with the conditions.

Heading into a dementia diagnosis without some help and support can be like setting out on a long journey to an uncharted place without a map or GPS and insufficient rations to sustain you on what could well be a long and challenging voyage. Finding yourself, spouse, partner, child, or a family friend, thrown into the role of ‘carer’ with little understanding as to what this might entail, can be very daunting.

We know that receiving such life changing news can sometimes rock the relationship boat. But finding accessible care and support at the right time can help you all to find balance and the space and time to find new ways of doing things so that life does move forward, albeit on a slightly different course.
For all affected, with the correct kit and provisions and a well rehearsed crew in support, you can undertake the journey with good prospects.

What do we do?

We support those living with various types of brain ill-health, and memory concerns including neuro cognitive disorder, Parkinsons and the various forms of dementia including Alzheimer’s, vascular cortical and sub cortical. We support early onset dementia and increasingly work with people in their late forties and early fifties.

Dementia Community links

This person centred service promotes and enables good health, well being and quality of life for residents of Preston and South Ribble, with dementia or targeted memory concerns. If you are under 65 with a dementia diagnosis, or older with or without a formal diagnosis, or you are a carer in these circumstances, then this service is for you.

This free, NHS commissioned, service is especially helpful if you are in transition following an initial diagnosis when the focus will be on the maintenance or re-development of skills.

The focus is on individual enablement, skills maintenance and support through a period of change, with the objective of supporting you to access mainstream interest groups and ultimately confidently and independently step out of service. You can expect the following:

  • Practical support to maintain or re-develop daily living skills
  • Support to undertake social pursuits
  • Maintaining a community presence
  • Activities – creative and educational
  • Sign-posting, advice and information on complementary servicesFor Carers this services is a route to finding support and solidarity through:
  • Useful advice and guidance
  • Chance to meet other carers, share experiences, building lasting support networks
  • Much needed respite from your caring responsibilities

FAQs

  • How do I get on board?

    Access to the six month service is via an open referral process. That means referrals can be made by yourself, or a family member, or by any health and social care practitioners or wellbeing and other services/agencies involved in the provision of care and support to older adults in South Ribble and Greater Preston.

    Referrals received are assessed to confirm the need for the service and appropriate level of support. This is done on a priority of need basis and your first appointment will follow on basis of priority of need.

    How long will it be before I receive an assessment appointment?
    It takes two weeks from referral to assessment. We impress the benefits of referring sooner rather than later. The earlier we can implement tailored individualised help and support the better your wellbeing outcomes, including your ability to adapt to the life changes up ahead.

  • What does the service look like?

    We allocate an officer to work alongside. You will identify your needs and wants and we will work with you to create your individualised action plan and prioritise your wellbeing goals, which will be your toolkit for moving forward.
    The service is mainly provided to you in your own home, but we also use community settings where appropriate, to access activities and socialising opportunities. Getting out and about is a good way to maintain or improve physically mobility and be better connected in with your local community and communities of interest, providing you longer term prospects to live as well as you can.

    We also support your wider family with ‘wrap-around’ practical advice and guidance.

    The service operates Monday – Friday excluding Bank Holidays.

  • How do I step off?

    Discharge Criteria and Planning
    We support your holistic care and wellbeing and will also signpost and refer you into other services and agencies, introducing you to the appropriate care pathway.

    After 6 months, to ensure a smooth transition, personalised appropriate discharge planning is undertaken prior to your leaving our care service.

  • What is Dementia Day Support?

    Day support offers a wonderful opportunity for you to continue living your life to the fullest. As a preferred provider for Lancashire County Council, our day centers—Withy Trees in Fulwood and Charnley Fold in Bamber Bridge—provide safe and enriching environments for learning and fun. This enables you to maintain your independence and stay connected to your home and community for longer.

    Staying active and engaged, both emotionally and intellectually, is essential for a high quality of life. Through our twelve-week structured therapeutic program, you’ll develop adaptive coping strategies, enjoy various activities and outings, and embrace new challenges alongside others. Reminiscence sessions help you preserve muscle memory and strengthen your connection to personal and community history.

    For caregivers, day support offers essential respite time, allowing you to recharge, pursue your interests, and connect with others. We facilitate peer support groups, providing opportunities to learn from and connect with fellow caregivers facing similar challenges.

    There are many aids that can help you to navigate your journey. Ideas to help you manage your way through daily life inside and outside your home, to socialise and have some fun along the way.

  • What support do you offer for families and carers?

    Regenerage recognises the vital role that family and carers provide in supporting people with dementia and memory concerns to navigate the ‘system’ and to get the help needed.

    At Regenerage, family members and carers are fully supported from advice and information and carers groups through to joint activities within the day centres.
    Our carers group takes place the first Wednesday of the month at Charley Fold, everyone is welcome.
