Social Prescribing

Over the last three years your local Primary Care Network (PCN) has partnered with Age Concern Central Lancashire to deliver Social Prescribing services in Preston and South Ribble.  Age Concern Central Lancashire has delivered social prescribing in many forms and over many years and indeed was one of the early pioneers of social prescribing in Lancashire.  The Age Concern Social Prescribing Team are employed in non-clinical roles and are recruited for their listening skills, empathy and ability to support people through non-clinical interventions.

Our Team can:

  • Assess how far a patient’s health and wellbeing needs can be met by services and other opportunities available in the community.
  • Co-produce a simple personalised care and support plan to address the patient’s health and wellbeing needs; introducing or reconnecting people to community groups and statutory services.
  • Provide personalised support to individuals, their families and carers to take control of their health and wellbeing, live independently and improve their health outcomes.
  • Develop trusting relationships by giving people time and focus on ‘what matters to them.’
  • Take a holistic approach, based on the person’s priorities, and the wider determinants of health.

On average, our team deliver between 6-12 contacts with a patient, depending on their needs, over a three-month period.  They connect people to community groups and help the patient to develop skills, friendships and resiliency.  Our Social Prescribing staff emphasise the strengths and capacities of local communities, enabling local groups and enterprises to receive social prescribing referrals.

  • What is a link worker

    Social prescribing link workers help to reduce health inequalities by supporting people to unpick complex issues affecting their wellbeing. They enable people to have more control over their lives, develop skills and give their time to others, through involvement in community groups. Link workers visit people in their homes, where needed.
