Community Links - Information & Support

If you’d like to know more about memory loss and dementia then Age Concern’s Information and Advice service could be the help you need.

Support is available through Age Concern day centres and our ‘Community Links’ staff work throughout local neighbourhoods and within people’s homes to offer information and advice in regards to memory loss, the symptoms of dementia and how to seek help.

Community Links is a free service and offers short term support, both on a one to one basis and through group settings.  It’s aim is to reduce social isolation, promote independence and to boost physical, social and emotional wellbeing.  It also helps to build confidence through linking people in to positive activities and community settings.

The service offers home visits to assess individual need and to provide person centred information and support.  Help is given to people suffering from memory loss in addition to their network of family, friends and carer(s).  Community Links connects people to local activities and groups as well as Age Concerns day support centres Charnley Fold and Withy Trees.  Help is also provided to access the centres and staff will accompany people if required.

Community Links also deliver group activities including; gentle exercise, dominoes and crafts in addition to supporting existing community led projects.  The service will also identify gaps within the local community and through volunteer support create new groups as needed.

All our Dementia Community Links Team have received ‘Covid Infection Control’ Training and wear full personal protective equipment  (PPE) whilst working.  This protects both our staff and you, keeping everyone safe whilst still being able to provide this essential service.
